Admission & Registration

Your consulting doctor or the clinical staff will make the necessary arrangements for your admission into the hospital. Upon confirmation of admission, you will be given an admission registration form to fill in, indicating your personal particulars, particulars of a family member or relative, payment mode and others.

You should bring along the following for your admission:

• Identity card/passport/birth certificate/CPF number
• A Set of Home Wear/Clothes
• Slippers
• Toiletries (e.g. toothbrush, toothpaste)

Note that a set of towel and pyjamas are provided by the hospital.
